process to submit a request for a credit file security
freeze, placement, temporary lift, or removal of a freeze.
To complete the process, you will receive an email from
Teletrack requesting that you provide additional
information to verify your identity. If you do not
provide this information, Teletrack will not be able
to place or remove the security freeze on your file.
If you prefer, you may always request a security
freeze placement or removal on your file by mailing
in your request, please complete the information above.
Print and complete the Freeze Request Form that will be
sent to your email address along with the requested
